Blue Ridge Independence at Home

Blue Ridge Independence
at Home is a Program of
All-Inclusive Care for the
Elderly (PACE) Center that
helps seniors who need
support remain safely and
independently in their homes.
Although developed by
Blue Ridge Hospice, it is not
limited to hospice care, but a
comprehensive, coordinated
healthcare program delivered by an interdisciplinary team. By focusing
on preventive care and long-term management of chronic conditions,
the program helps participants avoid nursing home placement and
continue living in their communities.
“Blue Ridge Independence at Home represents a powerful extension
of care; one that allows people to live longer, healthier lives in the
place they value most: home,” shares Mare Weiss, Principal-in-Charge.
The project supported the integration of hospice-rooted values into
PACE, helping strengthen dignity, continuity of care, and community
connections.
